As of January 2, 2024, I am joining Cleveland Clinic’s Obstetrics & Gynecology Institute. Mypractice location will remain at 809 White Pond Drive, Suite B, Akron, OH 44320.
Please be assured that you do not need to worry about transferring your medical records as they will be transferred with me. When you see me at Cleveland Clinic, your ongoing health records will be available in MyChart, offering you convenient online access to your health information, regardless of which Cleveland Clinic physician you see. To enroll in MyChart, or for more
information, please visit clevelandclinic.org/mychart.
If, after contacting your insurance company you still have questions or concerns about your insurance coverage, please call 216.445.1357. If you have an upcoming appointment, Cleveland Clinic may call you in advance to assist in the preregistration process and ensure your visit will be as smooth as possible. Please be sure to bring your driver’s license and health insurance card to your visit, and any co-payment due at time of service.
